One in Seven Users, $199 a Month, Zero Trials
Here is the pitch, and I'll grant you it's a good one. GLP-1 drugs like semaglutide are the most impressive pharmaceuticals of the decade. They don't just cause weight loss; in a 17,604-person trial they cut cardiovascular events by 20% and all-cause mortality by 19%.1 They lower inflammatory markers. They might slow biological aging. So why should the benefits be reserved for people with obesity? Take a fraction of the dose — a quarter, a fifth, a tenth — skip the nausea, keep your muscle, and collect the longevity dividend. A smaller needle. A longer life.
This is "microdosing," and it has moved from biohacker podcasts to a full-blown consumer category with remarkable speed. AgelessRx, a longevity telehealth company, sells a "Microdosing GLP-1" subscription from roughly $99 to $199 a month, marketed with claims that low doses "may reduce the risk of age-related cognitive decline" and "may trigger molecular pathways that help prevent heart disease, neurodegeneration, and metabolic disorders."2 Noom launched a microdose offering. Bryan Johnson, the internet's most quantified man, told a podcast audience he microdoses Ozempic and predicted most people eventually will.3 Fox News ran a "should you microdose Ozempic?" segment. UK clinics report hundreds of patients on one-fifth doses.
The behavior is no longer fringe. In a December 2025 real-world survey of more than 60,000 people by the health data company Evidation, about one in seven injectable GLP-1 users reported having microdosed — and the most commonly cited source of information about how to do it was social media.4 Roughly one in eight American adults now reports using a GLP-1 drug.5 Even a minority behavior within that population is millions of people improvising their own dosing.
So the question for this issue is narrow and specific. Not "are GLP-1 drugs impressive?" They are. The question is whether taking a sub-therapeutic dose of one, as a healthy person, for "longevity," has evidence behind it. I went looking. The published human trial count for that claim is zero.
What the Full Dose Actually Earned
To understand why this trend is so persuasive, you have to appreciate that the evidence it borrows is real. The SELECT trial, published in the New England Journal of Medicine in 2023, randomized 17,604 adults with cardiovascular disease and overweight or obesity — but no diabetes — to weekly semaglutide 2.4 mg or placebo for a mean of 40 months. Semaglutide reduced major adverse cardiovascular events by 20% and death from any cause by 19%.1,6 That mortality result is the crown jewel of the entire drug class, and it is the number doing the heavy lifting in every microdosing pitch.
Adults with existing cardiovascular disease and BMI ≥27, no diabetes, randomized to semaglutide 2.4 mg weekly or placebo for a mean of 39.8 months.
Results: Major adverse cardiovascular events reduced 20% (HR 0.80, 95% CI 0.72–0.90); all-cause mortality reduced 19% (HR 0.81, 95% CI 0.71–0.93).1
Limitation: Funded by Novo Nordisk; tested only the full 2.4 mg dose in secondary-prevention patients. Says nothing about low doses in healthy people — which is precisely the population and dose microdosing targets.
The inflammation story is real too, and it's the mechanistic hook the marketing leans on hardest. In exploratory analyses of SELECT and the STEP trials, semaglutide cut high-sensitivity C-reactive protein, a blood marker of inflammation, by roughly 40% versus placebo — and some of that reduction appeared early, before meaningful weight loss, and even among participants who lost less than 2% of body weight.7,8 That is genuinely interesting. It suggests GLP-1 receptor agonism has anti-inflammatory activity that isn't purely a weight-loss byproduct. It is also an exploratory biomarker analysis at the full dose, funded by the manufacturer, and a falling CRP is not a longevity outcome. Statins lower CRP as well; we do not conclude that a quarter-tablet of atorvastatin extends healthy lifespan in fit 40-year-olds.
Then there's the aging-clock result. In 2026, a UC San Diego team published a secondary analysis in Nature Communications showing semaglutide slowed the DunedinPACE epigenetic aging clock by about 9% over 32 weeks.9 Impressive sounding, until you read the fine print: full-dose semaglutide, in adults with HIV-associated fat redistribution, using a biomarker that has never been validated as a surrogate for actual lifespan. Longtime readers will remember Issue #054, where the same category of epigenetic clock endpoints propped up an entire consumer testing industry.
The hype is built on a real foundation. That's exactly what makes it effective.
On the microdosing pitchThe Dose-Response Data Points the Wrong Way
Every claim above shares one feature: the dose. SELECT used 2.4 mg. The CRP analyses used 2.4 mg. The epigenetic clock study used full therapeutic dosing. Microdosing protocols typically hold patients at 0.25 to 0.5 mg — the starter doses designed to be titrated through on the way to a therapeutic dose. The entire trend rests on the assumption that the benefits survive a 75-90% dose cut. Nobody has tested that. And the data we do have on dose and outcomes points in the opposite direction.

Results: Patients who reached ≥1.7 mg had roughly 58% lower all-cause mortality and 49% lower rates of death, heart attack, or stroke than those who stayed at 0.25–1.0 mg. Benefits tracked attained dose more closely than weight lost.10
Limitation: Observational — sicker patients who tolerate only low doses may do worse for other reasons. But note the direction: every dose-response signal that exists suggests benefits climb with dose. Microdosers are betting the other way.
What positive low-dose human data exists? I found one randomized trial worth naming. In JAMA Psychiatry in 2025, Hendershot and colleagues gave low-dose semaglutide, 0.25 to 1.0 mg, to 48 adults with alcohol use disorder and found reduced laboratory alcohol consumption and craving.11 That's a genuinely intriguing result. It proves low doses are biologically active in the brain. It is also 48 people, nine weeks, and an endpoint that has nothing to do with living longer.
Meanwhile, the flagship neurological claim took a direct hit — at the full dose. The observational studies behind "GLP-1s prevent dementia" were always vulnerable to healthy-user bias, but they were at least directionally encouraging: one 2025 cohort of over 60,000 patients found a 37% lower dementia hazard with semaglutide or tirzepatide versus older diabetes drugs.12 Then Novo Nordisk ran the actual experiment. The phase 3 EVOKE and EVOKE+ trials gave oral semaglutide to 3,808 people with early Alzheimer's for two years. Both trials failed their primary endpoint. Cognition did not improve. Novo discontinued its Alzheimer's program.13

Results: No difference on the primary cognitive endpoint (CDR-SB) or key secondaries, despite favorable biomarker movement. Program discontinued.13
Limitation: Tested treatment of established disease, not prevention in healthy adults. But if the full dose can't slow cognitive decline in two years of randomized testing, a fifth of the dose "may reduce the risk of age-related cognitive decline" is not a claim the evidence supports.
There's one more inconvenient piece of arithmetic. A major selling point of microdosing is avoiding the lean muscle loss seen with full-dose therapy — in the STEP 1 body-composition substudy, roughly 40% of total weight lost was lean mass.14 But that muscle loss scales with weight loss. A dose low enough to cause no weight loss plausibly also causes none of the benefits that tracked weight and dose in every outcome trial. The marketing wants the dose-dependent benefits without the dose-dependent effects. Pharmacology rarely offers that deal.
The only registered microdosing-for-longevity trial plans 150 participants, is run by the company selling the product, and uses surveys as its primary readout.15
A Trial of the Product, by the Product's Seller, on the Product's Customers
Whenever I say "there are no trials," someone sends me the trial. So let's look at it. NCT07092605, titled "Effectiveness of Microdosed GLP-1 in Improving Health, Quality of Life, and Longevity Measures," is the only registered study of this practice. Its sponsor is AgelessRx — the company selling the subscriptions. Its principal investigator is Sajad Zalzala, AgelessRx's co-founder and chief medical officer. Its inclusion criteria require participants to be new or existing AgelessRx patients. It is single-blind, aims for about 150 participants split across three arms, and runs entirely by telemedicine.15
The endpoints deserve their own sentence. Primary outcomes are surveys every four weeks; secondary outcomes are routine blood panels every twelve weeks and optional wearable data. No epigenetic clocks. No cardiovascular events. No mortality. Nothing in the design measures longevity, which is nonetheless in the title. One arm tests sublingual semaglutide, a route of administration with essentially no published bioavailability data for a peptide of this size — swallowed peptides are digested, which is why oral semaglutide required a special absorption enhancer and 14 mg to match a fraction of the injectable dose.

Endpoints: Quality-of-life surveys every 4 weeks; standard labs every 12 weeks; optional wearables. No aging biomarkers, no clinical outcomes.15
Limitation: Sponsored by the seller, led by the seller's co-founder, enrolling the seller's customers, measuring soft endpoints, using compounded product. Whatever this produces, it will be marketing collateral before it is evidence.
I want to be fair to the underlying idea: running a trial is better than not running one, and cost and tolerability are legitimate reasons clinicians sometimes use lower doses in patients who have an actual indication. That's the position Anne Komé and colleagues, including John Buse, one of the most cited diabetes researchers alive, took in a 2025 Diabetes Care commentary: micro-dosing may have a role for affordability and side-effect management in diabetes care, but there is no efficacy or safety evidence for the practice, and none at all for longevity.16 The distance between "a pragmatic workaround for patients who need this drug" and "a $199-a-month life-extension subscription for people who don't" is the distance this industry is monetizing.
There is no rigorous scientific data to support microdosing. Save your money.

Here's the part of the story that gets left out of the longevity podcasts: what's actually in the syringe. Brand-name semaglutide pens come in fixed doses, so most microdosing protocols run on compounded semaglutide — pharmacy-made copies in multi-dose vials. During the 2022–2024 shortages, compounding was legal at scale. But the FDA declared the tirzepatide shortage resolved in December 2024 and semaglutide in February 2025, and the enforcement grace periods ended in early 2025.17 Mass-produced copies are no longer permitted; the FDA clarified in 2026 that near-copies of commercial strengths don't qualify for the "personalized dosing" exemption either. The microdosing economy now lives in precisely that gray zone — prescriptions written for non-standard doses, in part because non-standard is what remains sellable.
The safety record of that supply chain is not theoretical. In July 2024, the FDA published an alert documenting hospitalizations from dosing errors with compounded semaglutide — patients drawing 5 to 20 times the intended dose from multi-dose vials, usually from milligram-to-milliliter conversion mistakes.18 Utah's poison control center logged a nearly fourfold year-over-year rise in semaglutide exposure calls.19 In September 2025 the FDA sent warning letters to more than 50 online sellers of illegally marketed GLP-1 products.20 This is the ecosystem in which one in seven users is improvising sub-therapeutic doses with guidance sourced from TikTok.
And the low dose itself buys less safety than the branding implies. GLP-1 class risks — gallbladder disease, pancreatitis, the rodent thyroid C-cell tumor signal that puts a boxed warning on every semaglutide label — have no established "safe floor," because sub-therapeutic dosing has never been formally studied in healthy people. Unknown benefit at an unknown dose with an unquantified risk profile, purchased from a legally ambiguous supply chain: in dermatology we have a word for this configuration, and the word is not "longevity protocol."

Post-Shortage Gray Market
Compounding exemptions ended in 2025. Over 50 FDA warning letters followed. "Personalized" microdose prescriptions are partly a legal workaround, not a clinical innovation.
No Low-Dose Safety Floor
Pancreatitis, gallbladder disease, and the thyroid C-cell warning were characterized at therapeutic doses. Nobody has established what risk remains at 0.25 mg — the studies were never done.
The Benefit-Risk Inversion
If the dose is too low to cause weight loss, the outcome benefits that tracked dose and weight in every major trial are plausibly absent too. You may be keeping the risks you meant to avoid and losing the benefits you meant to keep.
Real Drug, Real Data, Wrong Dose, Wrong Population
I keep coming back to how clever this pitch is. It doesn't ask you to believe in an unproven molecule — the molecule has some of the best outcome data in modern pharmacology. It asks you to believe the benefits survive being diluted out of the evidence base entirely, extrapolated from cardiac patients to healthy 45-year-olds, and delivered through a supply chain the FDA is actively policing. Each step sounds small. Together they span the full distance between medicine and marketing.
Full-dose GLP-1 therapy for people with obesity, diabetes, or cardiovascular disease is one of the genuine success stories of this era; that evidence is strong and I've said so in these pages. But "microdosing for longevity" is a different claim, and for that claim the published human trial count is zero. The dose-response data that exists points toward higher doses driving the benefits, not lower. The class's marquee brain-health hypothesis just failed two phase 3 trials at full dose. The only registered microdosing trial is run by the company selling the subscription, on its own customers, with surveys as the primary endpoint.
When the entire evidence base for a specific claim consists of borrowed data from a different dose in a different population, plus one seller-run survey study, the claim is marketing. This one is dressed better than most.
The mortality data microdosing borrows was earned at 2.4 mg in cardiac patients. At a fifth of the dose in healthy people, the trial count is zero — and the only study underway is run by the company selling it.
